A 6-Step Process is typically conducted which centers around

  • Gathering Information;
  • Analyzing Facts; and
  • Implementing Solutions.

After an accident occurs, the following STEPS should be conducted:

  1. Secure the areas where the accident occurred;
  2. Collect facts about what happened;
  3. Determine the root causes;
  4. Develop the sequence of events that occurred;
  5. Recommend improvements; and
  6. Report the facts about what happened and how corrections will be implemented.
