A 6-Step Process is typically conducted which centers around
- Gathering Information;
- Analyzing Facts; and
- Implementing Solutions.
After an accident occurs, the following STEPS should be conducted:
- Secure the areas where the accident occurred;
- Collect facts about what happened;
- Determine the root causes;
- Develop the sequence of events that occurred;
- Recommend improvements; and
- Report the facts about what happened and how corrections will be implemented.
